The valid ID you used to register, such as:
Unexpired West Virginia driver’s license
Unexpired West Virginia state ID (issued by the DMV)
Unexpired U.S. passport or passport card
For Minor Patients:
A registered caregiver must also be present with a valid OMC caregiver card and state-issued ID or passport.
